It's an elected monarchy if that makes any sense. Most Moroi are from royal families, but the term "royal" is used loosely in the society. Not all Moroi are royal, but most are. There are a few conditions for eligibility, but when the current ruler -- currently that is Tatianna Ivashkov -- steps down for whatever reason.... the families have to go through a process to be elected in and a series of tests.
